2/8/2024 EnchantaVaughn Approved Behavior: Client was observed in a civil manner and conducting herself responsibly in the home when QMHP arrived. Client demonstrated positive affection towards her mom and was seen laughing and playing with her little sister. Client appeared to be happy as evidenced by client smiling and displaying a cheerful attitude. QMHP engaged Ms. Spencer in a discussion about behaviors since the last session. She reports client struggles with being able to express her feelings reverting to aggression.. Behavior: Client followed directions, listened well, and stated that she enjoyed the activity that was being presented at this time. During the second portion of the session, mother reports client struggles with building healthy relationships with peers as well as client verbalizing to QMHP on how she gets bullied at school too sometimes which makes her upset. Intervention: QMHP inquired about what he looked forward to the most when coming to therapy. QMHP engaged client in conversation about some of the reports received from mom in relation to her mental health status. QMHP inquired about habits and routine at home when she begins her day. QMHP reflected on ways to push client to her full potential. QMHP assisted client in developing more social-emotional balance at home. QMHP challenged client to continue displaying a positive attitude while employing a positive relationship with her family. Intervention: QMHP encouraged mother to allow client to express herself in an age appropriate manner and to redirect her when necessary. QMHP modeled pro-social behaviors for client to strengthen her understanding of healthy interaction with peers and authority and to teach her team building skills needed for the school and community environments. QMHP provided support and encouragement to ensure client was given the proper attention needed to have a successful day. Response: Client and mother both shared that they were okay with today’s progress and expressed everything have been very informative regarding severity of her disorder. Client was able to utilize coping skills to eliminate stressors and decrease triggers related to her mental illness and promote a healthier well-being. Client also stated that she will implement each of the things she is learning in the session in her daily routine so that she is in a better position mentally and emotionally to be an asset to her family. Response: The mother shared that the client was doing so well with her involvement in the interventions and noticed a huge difference in her personality and mental state. Client is more outgoing and cooperative and is able to work well with others without making everything about her. Client continues to express herself in an appropriate manner and have utilized team building strategies to make new friends in the community and has a close bond with her mother and sister, indicating she understood her role in the home.
